File a Claim

Please accept our sincere condolences during this time. The loss of a loved one is difficult, and we are here to assist you in making this process as easy as possible.

There are several ways to notify us of a claim:

  1. Call us at 833-707-0012, option 1 and our customer service team will gather the information needed to begin the claim; or
  2. Email a copy of the death certificate, along with the completed claim form (see below), to [email protected]. If email is not an option for you, these documents can be mailed to us at:

What to expect when filing a claim

  1. Your information will be reviewed by our claim specialists.
  2. Once all requested information is received, your claim will be processed within 7-10 days. If additional information is needed, you will be notified.
